Medical Click Fraud: What to Watch For in Google Ads

Medical Click Fraud: What to Watch For in Google Ads

Medical click fraud is paid Google Ads traffic to a practice's site from bots, click farms, competitors or repeat clickers who will never book an appointment. The click is billed, the daily budget runs down, and the schedule stays the same. For a practice that pays a lot per click on local terms, a small amount of it adds up quickly.

Where Medical Click Fraud Comes From

It rarely arrives as one big event. It is usually a slow leak from a few kinds of source:

  • Competitor clicks. Someone who knows exactly which local terms you bid on and clicks to drain your budget early in the day.
  • Click farms. Paid people clicking from cheap devices, often in bursts.
  • Bots. Scripts that load the landing page and leave, or fill in a booking form with junk.
  • Repeat clickers. One person returning to the same ad many times over a few days.

Inside Google Ads all four look the same: a click, a cost, no appointment. That is why the checks below matter more than instinct. Our guide to repeat clickers in Google Ads covers the last group in more depth.

Warning Signs of Medical Click Fraud

None of these proves fraud alone. Two or three together, week after week, is the point to look closer.

  • Clicks rise while new patient enquiries stay flat or fall.
  • The budget is spent by mid-morning and ads stop showing for the rest of the day.
  • Clicks come from outside the area you serve.
  • Booking forms arrive with fake names, repeated numbers or nonsense in the fields.
  • Calls from ads are very short or never connect.
  • Clicks cluster at the same hours on the same device type in a way real patients do not.

Practices have a local advantage here. Most patients come from a limited radius, so a steady stream of paid clicks from far outside it is easy to question.

Checks You Can Run in Google Ads Today

ReportWhat a problem looks likeFirst move
Search termsClicks on terms unrelated to your servicesAdd negative keywords, review match types
LocationsClicks from places you do not serveTighten targeting to people in your area
Ad scheduleClicks when the front desk is closed, no bookingsCompare clicks and bookings by hour before changing bids

Invalid clicks columnA rising count on the same campaignsLook at your own click records for those campaigns

To add the invalid clicks column, open Campaigns, then Columns, then Modify columns, and search for "Invalid clicks". Those are clicks Google filtered before billing, so you were not charged for them. Treat the number as a floor. Then compare Google Ads clicks against the new patient enquiries your front desk logged that week. A wide and steady gap is the clearest sign.

Medical Click Fraud Is Not Healthcare Fraud

Healthcare fraud usually means billing schemes, false claims or stolen patient identities, and it belongs to compliance teams and regulators. Medical click fraud is an advertising problem: wasted paid clicks and junk booking requests. It does not touch patient records or billing, and the fixes are campaign settings, conversion tracking and exclusion lists.

How to Block Bad Sources

When a source is confirmed, exclude it so your ads stop showing to it.

  • Search campaigns. Campaigns, select the campaign, Settings, Additional settings, IP exclusions. Up to 500 addresses per campaign, and an asterisk can replace the last digits to exclude a block, such as 123.45.67.*.
  • Performance Max. Campaign-level IP exclusions are not available in Performance Max. Account-level IP exclusions apply to all campaigns in the account, including Performance Max, so add confirmed sources there.
  • Microsoft Ads. It has its own campaign-level IP exclusions with a lower limit. See Microsoft Ads click fraud for how that works.

Be careful with shared networks. Hospitals, offices and mobile carriers put many real people behind one address, and some of them are your patients. Exclude specific repeat offenders, not whole networks after one odd click. The Google Ads IP exclusions guide has more on keeping a list clean.

Refunds and What to Expect

Invalid activity Google finds after billing is credited as an "Invalid activity" adjustment, usually in the next billing cycle, under Billing, then Summary. If you think Google missed a pattern, the Click Quality Form covers the past 60 days. Send your customer ID, dates, campaigns, keywords and server logs with IP addresses and GCLIDs. Google will not tell you which specific clicks it judged invalid, and a credit is never guaranteed. See Google invalid click refunds for the full process.

A Weekly Routine for a Small Practice

  1. Compare clicks, cost and booked appointments for the past seven days.
  2. Scan the search terms and locations reports for anything outside your services or area.
  3. Note any source that repeats without producing an enquiry.
  4. Add confirmed sources to your exclusion lists, with the date and the reason.
  5. Once a month, review the whole list and remove entries that no longer belong.

Count a conversion when an appointment is actually booked, not when a form is sent. That one change stops Google Ads from chasing traffic that only fills forms.

Where Automation Fits

A weekly review catches the obvious patterns but struggles with volume, because someone has to look at every click. Automated click fraud protection checks each paid click as it arrives, flags bots, click farms and repeat clickers, and writes confirmed sources into your exclusion lists, including the account-level list that covers Performance Max. It cannot stop the very first click from a new source, and it cannot guarantee a Google refund.

If you want this done automatically, see how Block Click Fraud works.

Frequently Asked Questions

What is medical click fraud?

It is paid ad traffic to a medical practice from sources with no intent to book: bots, click farms, competitors and repeat clickers. You pay for each click and get no appointment from it.

Can I block IP addresses in a Performance Max campaign for my practice?

Yes, at account level. Campaign-level IP exclusions are not available in Performance Max, but account-level IP exclusions apply to every campaign in the account, Performance Max included.

Do IP exclusions stop medical click fraud completely?

No. They stop your ads showing to sources you have already identified. A new bot or a first-time clicker still gets through until it is spotted, so exclusions work best as a list you keep adding to.

Will Google refund clicks I think were fraudulent?

Google credits activity it judges invalid, and you can ask it to review the past 60 days with the Click Quality Form. A credit is not guaranteed, and Google will not confirm which clicks it ruled invalid.

How often should a practice review its ad traffic?

Weekly suits most practices: clicks, cost and booked appointments, plus a scan of search terms and locations. Review the exclusion list monthly. If the budget starts running out early in the day, look the same day.
