Do IP exclusions apply to the whole Google Ads account?

No. IP exclusions are a campaign level setting, so an address blocked on one campaign still sees ads from every other campaign in the account. There is no native account-wide exclusion list and no built-in way to copy one list across campaigns. Advertisers running many campaigns either repeat the work by hand or push the lists through the API with a script.
